Your medical records may be requested from the Health Information Management department, located across from the Gift Shop on the east side of Tucson Medical Center. Hours of operation are Monday through Friday 8 a.m. to 5 p.m., excluding major federal holidays. You can reach us at (520) 324-5165 or by email tmcmedicalrecordsrequest@tmcaz.com.

Please bring photo identification with you. If the records you need are not available electronically, most requests should not take longer than 7 business days. If it has been over 7 days please give us a call. However, factors such as record availability, size of the record and the volume of requests will affect response time.

Tucson Medical Center partners with Sharecare Health Data Services as our vendor for releases of information. Sharecare has the capabilities for releasing documentation through various modes. If you have questions please contact Sharecare at (520) 324-2252.

There may be a charge for the cost of reproduction of medical records billed by TMC or Sharecare. To obtain your records, you need to complete a Medical Records Release of Health Information Authorization Form, which is also available at Medical Records.

Radiology records

For films or digital images, please call (520) 324–5000. Authorization and release for information is required.

A top priority is to protect the security and privacy of patient information. All information contained within the patient medical record is considered confidential and is federally protected under the Health Insurance Portability and Accountability Act of 1996, or HIPAA. Medical records shall not be released without the written authorization of the patient, his or her legal representative, a subpoena or court order.
